GROUP HOME LIFESTYLE PROGRAM
Group Home – Lifestyle Program offers:
- Experienced and qualified House Managers and Direct Care Workers employed at community group homes at 8 locations within the Sydney and Nepean areas.
- Individual Planning for Lifestyle & Recreation, Health, Medical, Para-Medical and Personal Care according to SNAP Assessment identified level of support need.
- Pastoral Care available for families in their own home.
- Individual Assessments and Individual Plans for Speech & Communication, Behaviour Intervention, Audiology, Ophthalmology, Dysphagia, Epilepsy, Mobility , Occupational Therapy, Podiatry, Orthotists, Psychology, Psychiatric, Physiotherapy, Respiratory, Sleep Apnoea, Health & Medical, Dietary and Nutrition.
- Individual Finance Management Plan for management, supervision and accountability of money, individual cheque accounts, purchase of personal items, equipment items, medical and other accounts payable.
- Family Relationships’ building workshops.
- Advocacy and Guardianship Orders co-ordination and compliance.
- Management support, coordination and attendance at all appointments with Specialists and Medical Practitioners, including support for all periods of stay in hospital, to ensure consistency of information at all times to meet individual needs.
- Direct care supervision and transport to all appointments.
- Trans-disciplinary IT and Email approach by Management and Direct Care Workers to Group Home Daily Routines at 8 locations.
Group Home – Recreational Program offers:
- Weekend activities, picnics, shopping, movies and family visits.
- Holidays within NSW, interstate and overseas arranged with in-house support staff or with external tour group support.
Grunsell House – Recreational Program offers:
- Forsight Foundation Alumni, Social Club and Dinner Group.
- Dance Therapy with a qualified Dance Therapist and Psychologist.
- Art &Craft
- Multi-Sensory Room
Community - Recreational Program offers:
- Blind Bowls
- Blind Cricket
- Riding for the Disabled
- Deaf Blind Association - Hand over Hand Club.
- RAPS
Day Respite Program
- Day respite care program with appropriate level of direct care is offered to adults who are unable to attend day programs or supported employment services due to illness.
- Transport is provided to adults for pick up from day program or supported employment service due to illness during the day.
One-to-One Support
- One-to-one support is available for adults who need to have time-out from their group home due to health or individual need. Accommodation at Grunsell House with one-to-one staffing support for an agreed period is available.
Vocational and Employment Programs
- Day Programs and Supported Employment Services offered throughout Sydney metropolitan and Nepean area and accessed at 12 different locations in the Sydney metropolitan and Nepean area, including daily transport and taxi.
- Forsight Foundation Retirement Program offered to adults who do not attend Day Program or Supported Employment Services 5 days per week.
Professional Staffing
- Basic qualification - Certificate IV level in Disability Studies to Tertiary Qualifications in a related field with at least 2 years full-time study completed or higher.
- Quarterly in-house compulsory staff training program.
- External staff training program.
- Life Coach and Counselling.
- 24-hour/ 7 days Advocacy provided to all staff and residents.
- Networking with and membership of Disability Peaks and Groups.
- Registered Nurse Consultant on staff.
- Psychologist and qualified Dance Therapist on staff
- Local Medical Practitioner – General Practitioner Advisory Service to Management.
Board of Directors and CEO
- The Board consists of a qualified and experienced team of 10 volunteers who meet monthly to provide Corporate Governance to the organisation, oversee the Financial Management and Administration and the Group Homes and monitor their delegation of operations to the CEO.
